OBJECTIVES: In this study, we investigated the effects of intense pulsed light (IPL) combined with 30% supramolecular salicylic acid on facial seborrheic dermatitis. METHODS: A total of 45 patients with mild or moderate facial seborrheic dermatitis were selected from our hospital between September 2018 and September 2019. The patients were divided into three groups consisting of 15 patients each. The first group was exposed to a combination of IPL and 30% supramolecular salicylic acid treatment, the second group was exposed to the IPL treatment alone, and the third group was exposed to the 30% supramolecular salicylic acid treatment alone. They were treated once every 4 weeks in three consecutive rounds. RESULTS: Facial lesions and symptoms were observed 4 and 12 weeks after the first treatment, and adverse reactions were recorded. The combination group showed significant improvement in symptoms 4 weeks after the first treatment, while the individual treatment groups showed no significant improvement. After three rounds of treatments, seborrheic dermatitis had significantly decreased in the three groups; the efficacy of the combined treatment group was significantly higher than that of the IPL group and the 30% supramolecular salicylic acid group. CONCLUSION: IPL combined with 30% supramolecular salicylic acid was effective in the treatment of facial seborrheic dermatitis and provided a quicker result with no adverse reactions.

RESUMO

Dermatite seborreica (DS) é uma afecção comum da pele, caracterizada por lesões eritêmato-descamativas que afetam principalmente o couro cabeludo e a face. Sua prevalência em populações sadias varia de 2% a 5%, segundo diferentes estudos. Acomete ambos os sexos, sendo mais comum em homens. As lesões se desenvolvem em áreas ricas em glândulas sebáceas, como couro cabeludo, face, pavilhão auricular, flexuras e tórax.A dermatite seborreica da face acomete sobrancelhas, base dos cílios, pregas nasolabiais e o canal auditivo externo e pode ser tratada com corticoide tópico, metronidazol e antifúngicos tópicos, sendo o cetoconazol o principal.

Seborrhoeic dermatitis [SD] is a common skin disorder. Malassezia yeasts have an important role in the etiology of SD. Since anti-fungal agents, especially in azoles are effective for treating SD, in this study, the effect of ketoconazole 2% solution on clinical signs and Malassezia in SD patients were assayed. 100 patients with SD were enrolled in this study. Patients were scored in regard to the severity of lesions at the initial evaluation and every 2 weeks for a 1 month period. Microscopic examination and culture of patients scale in days 0 and 28 were used for isolation and identification of Malassezia species. Patients were divided into two groups [ketoconazole 2% solution and shampoo] and followed after 14 and 28 days, and then clinical response was graded. 58% of patients showed lesions on their heads. In day 0, 51% of patients showed > 7 yeasts in each microscopic field. 77% of scale samples were positive to Malassezia spp. Growth and M. globosa [57.1%] had the most frequency. In day 28, 89.6% and 82.6% of treated patients with solution and shampoo showed 1-3 yeast in within entire smear, respectively. 94.8% and 82.6% of scale samples were negative to Malassezia spp growth, respectively. In day 0, patients with moderate SI had the most prevalence, whereas in day 28, patients with mild SI were predominant. Statistical test showed the correlation is significance only between SI and treatment with solution. The results of our study showed that according to decrease of yeast load and increase of improvement of SD signs after treatment with ketoconazole 2% solution, compared with ketoconazole 2% shampoo, 2% ketoconazole solution can be considered as an appropriate agent in treatment of Sd

La dermatitis seborreica del cuero cabelludo va desde una pitiriasis simple difusa a costras eritematoescamosas. La inflamación y el Pytirosporum ovale juegan un papel importante, por lo que los corticoides tópicos resultan en general exitosos. En el presente estudio se analizan la efectividad y seguridad del uso de clobetasol al 0,05 por ciento en formulación champú, aplicado durante 15 días, por 27 pacientes con dermatitis seborreica moderada o severa. La efectividad fue evaluada a través de analizar prurito, descamación y eritema. La seguridad se evaluó constatando irritación local, eritema y descamación en áreas vecinas. Esta formulación es altamente efectiva, con desaparición del prurito en 92 por ciento de los pacientes a los 15 días de tratamiento. Similar situación ocurre con la disminución del eritema. Respecto a seguridad, en un 4 por ciento de los pacientes advirtió irritación local.


Seborrheic dermatitis of the scalp can manifest itself as anything from a simple diffused pityriasis to erythematous plaques. In all of these, the degree of inflammation and the presence os Pityrosporum ovale play an important role. We present an analysis of the effectiveness and safety of clobetasol shampoo formulation, applied daily for 15 minutes during 15 days, in 27 patients suffering from moderate or severe seborrheic dermatitis. Effectiveness was evaluated through analyzed pruritus, scaling, and erythema. Safety was evaluated by verifying irritation in the area of application, erythema and scaling of surrounding areas. Results were highly effective: pruritus disappeared in 92 percent of patients after 15 days, and erythema decreased in similar terms. Only 4 percent of patients presented local irritation.

Pimecrolimus cream 1% has shown to be effective in patients with a variety of inflammatory cutaneous disorders. And it might be a useful modality in the treatment of seborrheic dermatitis. This prospective study was aimed at assessing the efficacy and tolerability of pimecrolimus cream 1% in the treatment of facial seborrheic dermatitis. Twenty patients were instructed to apply pimecrolimus cream 1% for 4 consecutive weeks. Assessment of the disease severity was performed at baseline and at week 1, 2, and 4. Clinical assessments of erythema, scaling, and pruritus were measured using a 4-point scale (0-3). Global assessments of the disease severity by patients and investigators were performed at each visit. Mean clinical scores of erythema, scaling, and pruritus significantly improved by 87.4%, 91.9%, and 91.5% respectively at week 4 (p<0.001). Improvements in the global assessment of disease severity determined by patients and investigators also showed excellent results. No specific adverse events other than transient burning and tingling sensations were noted. The relapse of facial seborrheic dermatitis was mostly observed between 3 to 8 weeks after the discontinuation of pimecrolimus. We suggest that the topical application of pimecrolimus cream 1% can be an effective and safe alternative for treatment of facial seborrheic dermatitis.

BACKGROUND: Seborrheic dermatitis is a common, chronic inflammatory disease of the skin, characterized by erythematous plaques that are covered with yellow, greasy, scales and may or may not be associated with itching. Although a few studies have suggested use of topical metronidazole for the treatment of seborrheic dermatitis, there is no general consensus about it. AIMS: To evaluate the efficacy of 1% metronidazole gel and its vehicle alone, in the treatment of seborrheic dermatitis. METHODS: This was a double-blind, prospective, clinical trial. Fifty six patients with seborrheic dermatitis were randomly allocated to either apply 1% metronidazole gel or placebo to their facial lesions, twice daily for 8 weeks. All the patients were evaluated and scored every 2 weeks for 8 weeks. The results were analyzed statistically. RESULTS: A total of 56 patients entered this study and 53 patients completed the treatment course. There was no significant difference in terms of demographic data and lesions severity score. There were statistically significant differences in the reduction of mean severity scores between the 2 groups at the second, fourth, sixth and eighth weeks of treatment (p<0.05). Metronidazole gel significantly decreased mean of seborrheic dermatitis severity score from the 2nd visit (p<0.001). CONCLUSION: Metronidazole gel is an effective treatment for facial seborrheic dermatitis.

OBJECTIVE: To demonstrate the effects of a new biphasic oral contraceptive (Oilezz) on cycle control as well as mild to moderate acne and facial seborrhea of healthy fertile Thai women. MATERIAL AND METHOD: The trial is a prospective, open, non-comparative, single center study. Fifty healthy, fertile Thai women with mild to moderate facial acne were recruited to study a specific drug (Oilezz) for 6 months. RESULTS: At the beginning, 66% of the subjects had mild acne and 34% had moderate acne. Significant improvements in facial seborrhea grades (as indicated by Sebutape assessments) were found after the first cycle. These improvements increased steadily and were much larger after the sixth cycle. There were no statistically significant changes in body weight or blood pressure during the study. No serious adverse events were reported. There were no mood changes, migraine, rash, abdominal discomfort, malaise, nausea and decrease in libido during the study period The premenstrual symptoms at initiation were 21 cases (42%). The symptoms were 4 (8%) with headache, 8 (16%) with breast tenderness, 5 (10%) with dysmenorrhea and one (2%) with bleeding irregularity. These symptoms were improved in the third and the sixth cycles. The percentage of women with spotting or bleeding increased after first cycle, compared with baseline and gradually decreased during subsequent cycles. After the sixth cycle of treatment, all subjects had improvement of acne. 80% of cases recovered from acne and there were only 20% had mild acne. CONCLUSION: Facial seborrhea and acne improved significantly with Oilezz. It is good to control cycle without change in body weight and blood pressure. Therefore, Oilezz can be used for treatment of seborrhea and acne and as a contraceptive.

OBJECTIVE: To compare efficacy of 2% ketoconazole cream with 1% hydrocortisone cream in the treatment of infantile seborrheic dermatitis (ISD). This study recruited infants of 2 weeks to 2 years old with a clinical diagnosis of infantile seborrheic at the Department of Pediatrics, King Chulalongkorn Memorial Hospital from December 2001 to November 2003. METHOD: The severity of the rash of the patients was examined in terms of erythema, scale and crust before treatment based on a scoring system comparing two sides (left and right) of the lesion with numeral representation for it a severity, namely: Ofor no lesion; 1, mild; 2, moderate; and 3, severe. They were treated with 2% ketoconazole cream on the left side of the lesion and 1% hydrocortisone cream on the right side of the lesion, twice a day. RESULTS: The responses of treatment of 2% ketoconazole cream compared with 1% hydrocortisone cream in 48 patients had no statistical significant difference at 2-3 days of treatment and 4-7 days of treatment. By one week, both 2% ketoconazole cream and 1% hydrocortisone cream have significant clinical improvement of the lesion. The skin lesions were cleared in 31% of the ketoconazole group and 35% of the hydrocortisone group. All skin lesions were cleared by the end of the second week. CONCLUSION: The efficacy of 2% ketoconazole cream and 1% hydrocortisone cream in the treatment of ISD was not significantly different. Ketoconazole is another option for the treatment of ISD, to avoid the side effects of topical corticosteroid in long-term use and on large surface area of treatment.

RESUMO

HTLV-1 infection is endemic in the Caribbean and several publications have reported the clinical disease entities seen in this population of patients. This case report is an account of a patient admitted to Kingstown General Hospital, St Vincent and the Grenadines, who had severe infective dermatitis, tropical spastic paraparesis (TSP) and Non-Hodgkin's Lymphoma (NHL). As far as we are aware, all three diseases have not been described in a single patient

FUNDAMENTOS - A corticoterapia tópica está indicada no tratamento de lesöes eczematosas, como dermatite atópica e dermatite de contato, e de lesöes eritêmato-escamosas, como a psoríase e a dermatite seborréica. A busca de alternativas mais eficazes e mais bem toleradas é plenamente justificável. OBJETIVOS - Comparar a eficácia e a tolerabilidade de um corticosteróide tópico de média potência - o creme de fuorato de mometasona a 0,1 porcento, aplicado uma vez ao dia - em relaçäo a um de baixa potência, o creme de hidrocortisona a 1 porcento, aplicado duas vezes ao dia. MÉTODOS - O estudo clínico incluiu 48 pacientes, de ambos os sexos, com idade entre dois e 14 anos, portadores de dermatoses corticossensíveis, em bom estado geral. O estudo foi aberto, randomizado e prospectivo, com duraçäo de 21 dias. RESULTADOS - O creme de furoato de mometasona foi mais eficaz do que o creme de hidrocortisona na indicaçäo proposta (x=5,48;p<0,01). Näo houve diferença estatisticamente significativa entre os dois fármacos quanto à tolerabilidade. CONCLUSÄO - A mometasona tópica, sob a forma de creme a 0,1 porcento, em aplicaçäo única diária, mostrou-se estatisticamente superior à hidrocortisona, sob a forma de creme a 1 porcento, aplicada duas vezes ao dia, em termos de eficácia, sendo estatisticamente equivalente à droga de comparaçäo, no que se refere à tolerabilidade

En un estudio doble ciego y randomizado hecho en 35 pacientes (9 hombres y 26 mujeres) entre 1 mes y 13 años de edad, se comparó la efectividad de la aplicación de champú de ketoconazol al 2 por ciento aplicado sobre el cuero cabelludo 2 veces por semana por el período de un mes. Al inicio del estudio, en el 57,1 por ciento de los casos el cultivo de Pityrosporum ovale fue positivo. El grado de severidad de la enfermedad fue evaluado al inicio y al final del ensayo por intermedio de una escala ordinal. Los resultados demostraron una mejor respuesta del grupo con placebo en la reducción del eritema (p=0,002, test de Wilcoxon), de la descamación (p=0,002), y de la seborrea (p=0,04). Estos hallazgos se atribuyen al lavado sistemático y a las propiedades detergentes de la base empleada durante la duración del ensayo. El ketoconazol tópico aplicado en la misma base detergente no fue superior que el placebo en el tratamiento de la dermatitis seborreica del cuero cabelludo en niños

Dois tercos da populacao sofrem de dermatite seborreica mais ou menos grave, uma doenca cutanea associada com a levedura Pityrosporum ovale. Esse trabalho demonstra a atividade antifugica de duas substancias usadas comumente nas formulacoes para cabelos: o piritionato de zinco e a piroctona olamina em varias concentracoes. Com base em uma concentracao otima, foram formulados dois xampus, para determinar a tolerancia cutanea dos produtos acabados. Foi realizado um estudo de perfilometria, usando impressoes negativas obtidas de impressoes da pele, para calcular a tolerancia cutanea. Esse estudo revelou a superioridade da piroctona olamina como antifugico e a tolerancia cutanea a piroctona olamina tambem foi melhor

El uso de Ketoconazol en champú en el tratamiento de la Dermatitis Seborreica del cuero cabelludo se ha demostrado muy efectivo en el control de esta afección. Se trataron 25 pacientes, con un 80% de buenos resultados. Se describen otras formas terapéuticas para el tratamiento de la Dermatitis Seborreica del cuero cabelludo, comparándolas con el uso de imidazoles tópicos

Se trataron con isotretinoína, 32 pacientes -24 varones y 8 mujeres- que padecían acné nódulo-quístico severo. Los resultados fueron excelentes y muy buenos en el 90%de los casos, a una dosis de 0,5 mg/kg/día durante ocho semanas. Se observaron queilitis, sequedad de la piel y descamación general en la mayoría de los pacientes. No se comprobaron otras alteraciones biológicas

Se trataron 30 mujeres que presentaban manifestaciones clínicas de hiperandrogenismo, tales como hirsutismo, acné, seborrea o la asociación de ellos. Se les administró una combinación de 2 mg de acetato de ciproterona y 50 *g de etenil estradiol (Diane) del quito al vigésimo quinto días del ciclo menstrual, durante 6 meses. Al finalizar cada ciclo se evaluó el grado de hirsutismo, acné, seborrea y la aparición de efectos indeseables del medicamento. Encontramos una notable mejoría del acné en todas las pacientes y de la seborrea en la mayoría. También se demostraron efectos beneficiosos sobre el hirsutismo en la mayoría, aunque la mejoría no fue tan ostensible. Los efectos desagradables del medicamento fueron mínimos. No se produjo ningún embarazo, por lo que la eficacia contraceptiva fue del 100 %. Se concluye que esta combinación de baja dosis de acetato de ciproteron y etinil estradiol es muy útil en las mujeres con manifestaciones clínicas de hiperandrogenismo que deseen contracepción, pues por sus acciones tienen un doble efecto beneficioso

Os autores apresentam os resultados obtidos com a utilizaçäo de quatro formulaçöes de xampus no tratamento da dermatite seborréica e pitiriasis capitis, bem como o resultado dos exames bacteriológicos do couro cabeludo desses doentes. Os melhores resultados terapêuticos foram conseguidos com as formulaçöes contendo Zinco Piritione e Zinco Piritiones - Polytar. Näo foi observada qualquer reaçäo colateral com uso das formulaçöes
